Hoisting Propane Cylinders
Case History
Two 100-pound propane cylinders were to be hoisted from the ground to the roof of a multi-storey apartment building. The cylinders were laid horizontally on a hoisting platform that had a barrier edge around its perimeter.
As the platform with the two cylinders was being hoisted, a loose tagline attached to the base got tangled and caught, causing the platform to tip. The cylinders rolled to the edge, the perimeter barrier couldn't stop them, and they fell about 100 feet to the ground.
One of the cylinders split open on impact, releasing a cloud of propane that was ignited by a nearby electrical transformer. An explosion and fire resulted. The second cylinder was engulfed in the fire and exploded shortly afterwards.
Although no one was injured or killed, there was extensive damage to property caused by the explosions and ensuing 150-foot fireball.

Recommendations
When hoisting propane cylinders, always use a sturdy cradle or basket that allows the cylinders to be secured and maintained in an upright position. The hoisting platform must be designed so that each cylinder being hoisted can be adequately secured to it in an upright position. The platform must also be designed to safely support the greatest possible load to which it may be subjected.
